CAPT Kolstad was Commanding Officer USS Cochrane (DDG 21) from Jan 1967 to June 1968.  I recently came across the below.

Capt. Tom Irby Kolstad, USN (retired) died January 16, 2010 at his home in Redmond, Oregon. Burial on May 22 will be at Palestine City Cemetery, East Hill Section. He was born in Palestine, Texas, on July 13, 1928, the son of the late Chester Luckett Kolstad and Carrie Irby Kolstad. Capt. Kolstad attended the U.S. Naval Academy and served in the Navy for 25 years. After retiring from the Navy, he became an ordained minister for the Assemblies of God. He was preceded in death by his wife of 53 years, Carolyn Mears Kolstad on May 30, 2009. He was also preceded by his sisters, Sarah Kolstad Glass, and Freddie Louise Kolstad Hopgood. He is survived by his brother, Luckett Kolstad; and children, Tom Kolstad Jr., Elizabeth Leeberg, and Linda Anthony.
